Our services

Journey Hospice provides care that is specialized to your needs. Our staff focuses on providing compassionate care, education, and support for all our patients and their loved ones so they are as comfortable as possible. Journey Hospice has on call staff available 24 hours a day, seven days a week.

Nursing

Your primary care nurse will coordinate your care with other members of the interdisciplinary team and handle any nursing needs. Your nurse will help prevent and relieve pain and other symptoms, assist in monitoring medications and teach caregivers ways to provide for your care. The hospice nurse will try to anticipate your needs and assist you to better understand the natural progression of your illness.

Medical Social Services

Facing a serious illness can be a time of tremendous pain and confusion for you and your family. A medical social services professional will be available to help you and your family deal with emotions and challenges throughout your illness. Services may include:

  • Providing emotional support and guidance to the patient and family coping with the stress related to the illness.

  • Identifying community resources which are available to help you

  • Assisting the patient and family with planning for funeral arrangements, nursing home placement, etc.

  • Offering information about advanced directives.

Spiritual Counseling

Hospice spiritual care is based on a high respect for you and your caregivers personal faith and beliefs. The hospice spiritual counselor is available to assist you and your family as you cope with the significant spiritual issues that often arise during terminal illness.

Hospice Aide

Our hospice aides have been carefully chosen to provide care for our patients. The hospice aide assists with the patient’s personal care which may include bathing, hair care, shaving, skin care, and linen changes. They will report any changes in the patient’s condition to the appropriate team member.

Bereavement Services

Following a terminal diagnosis, you and your family may struggle with anticipatory grief issues. A professional is available to assist you and your family during this difficult time. Hospice continues to provide bereavement support to grieving families for a period of 13 months following the patient’s death.

Volunteer Services

A vital part of hospice is our volunteers. Volunteers provide many different types of support for the hospice program and those coping with terminal illness, grief and loss. They are good listeners, nonjudgmental, adaptable, and have a strong desire to reach out with love and concern to others.
